We receive a Placement Referral Form and work closely with the referrer to gather all relevant information about the young person's background, needs, history, and any specific placement requirements.
We conduct a thorough matching process — considering the young person's individual needs, risk profile, existing relationships, and the suitability of available accommodation and peer group dynamics.
We ensure each young person is welcomed warmly and given time to settle. A welcome pack, introduction to their keyworker, and a tour of their new home help establish a sense of safety and belonging from the outset.
Working with the young person, we develop an individual support plan that identifies their goals, strengths, areas for development, and the specific support they will receive. This plan is regularly reviewed and updated.
Through our Preparation for Independence programme and day-to-day support, young people develop practical life skills, improved confidence, better emotional regulation, and stronger community connections.
